Your Information, Your Rights

Our Fair Processing Notice explains why we collect information about you and how that information may be used.  To deliver your direct care and manage local health and social care system. 

The Notice Reflects:

  • What information we collect about you
  • How and Why we use that information
  • How we retain your information and keep it secure
  • Who we share your information with and why we do this

The notice also explains your right in relation to consent to use your information, the right to control who can see your data and how to seek advice and support, if you feel that your information has not be used appropriately.  Please see below link to view our Notice and to print if you wish.

Connect Care: How We Use Your Information

What is Connect Care?‚Äč

In the past health and social care workers from the local organisations involved in your care, did not have a system place to share important information about your care automatically.  This meant that GP's, Hospital Staff, District Nurses, Occupational Therapists and Others involved in your care, did not always have immediate access to all the information needed to provide timely and efficient care.

To address this in 2015, a new local electronic record system was launched, this system is called Connect Care.
