We’re committed to making sure you always receive the best possible care from our practice. To help us understand your health needs a little better and plan your care more effectively, we’ll be adding a simple care code to your medical record.

This code helps our clinical team quickly see the level of support you might need, so we can offer the right appointments, reviews, and follow-up at the right time. It’s just one of the tools we use to personalise your care and make sure nothing important is missed.

There’s nothing you need to do – we’re simply keeping you informed.

The care code is added automatically using information already in your medical record, such as any long-term conditions or regular treatments. It doesn’t affect your access to care in any way and doesn’t replace clinical judgement – it just helps guide us when planning your care.

You might see one of these descriptions in your record:

• General good health (135815002) – you’re generally well and only need routine care
• Chronic long-term disease management required (416239002) – you have one or more ongoing conditions that benefit from regular check-ups
• Chronic long-term disease management required: complex needs (416357003) – you may need a bit more coordinated or proactive support because of multiple or more complex conditions
